This means you have a one-time opportunity to apply for voluntary coverage without the usual medical

questions and other requirements.

Employees who take advantage of this opportunity for relaxed medical underwriting will also benefit

from affordable premiums, payable through convenient bi-weekly payroll deductions.

Everyone has different needs when it comes to insurance protection, and as your personal and financial

obligations grow, so does the need for appropriate coverage.   Through this new program, we are offering you

the ability to select the benefits you want, to create a package that fully meets your specific needs and those of

your family, in addition to your health benefits.

Note that the plans have minimum participation requirements, therefore, if there is insufficient participation,

coverage may not be issued or underwriting terms amended.
